Type CUI in the command line to open the Customize User Interface.

In the Customize tab, right-click on Toobars under the Customizations in All CUI Files category.   

Select New to create a new toolbar.  

Navigate  through the Command List to the command you would like to use. Once the command is highlighted you will see that the right hand side of the screen becomes populated with a selection of button images to choose from for your new toolbar.  You can modify an existing image or create a new one from scratch by selecting the edit button.

Once the new button has been created for the command, you will see an image of the button appear next to the command in the Command List.  

Click and drag the command into the new menu that you have started.  When you are finished, hit Apply and exit.
